Sports Physicals

Our physicians can provide sports physicals that are compliant with AHSAA guidelines.


We will check vital signs, vision, include a physical examination and discuss a past medical history form. If any additional testing is recommended, it will discussed upon your visit.


You can download the required sports physical form below. Please fill out the first sheet and bring it with you to your visit.
